The ScriptSave® Personalized Wellness Program is made available to you by Medical Security Card Company, LLC, a Delaware limited liability company ("MSC", "we", "us", and "our"). This Privacy Policy (“Policy”) describes our policies on the collection, use, and disclosure of personal information about you in connection with your use of the ScriptSave® Personalized Wellness Program, (the “Wellness Program”) which may occur via the MealAdvisor mobile application ("Application").

We are committed to safeguarding the Personal Information we receive from our customers and partners. We impose standards to maintain the confidentiality of Personal Information. And we use physical, technical, and administrative safeguards to protect it.

Personal Information is information that can be connected to you through certain identifiers which can include: name, Social Security number (SSN), demographic and geographic information such as city, county, state and zip code, latitude and longitude, IP address, driver’s license, state identification card, employer, health plan identification number, medical information, account, credit/debit card, passport or alien registration numbers — among other items. Personal Information does NOT include publicly available information that is lawfully made available to the general public from federal, state or local government records or widely distributed media.

Our policies prohibit the unlawful disclosure of Personal Information. We share it externally only where federal and state law allows or requires it. Internally, it’s our policy to limit the access, use and disclosure of Personal Information to be in line with the job duties our associates, as well as applicable law.

Personal Information may be collected from you directly and/or may be disclosed to us from third parties such as apps and websites that you link to the Application from. To the extent that we receive protected health information about you from Medicaid-Indiana, Inc. or its Affiliates (collectively, "Medicaid-Indiana"), such data will be disclosed pursuant to our partners’ Privacy Policy and HIPAA Notice of Privacy Practices and then governed by this Privacy Policy and applicable law. 

What information will we collect?

Information may be collected in the following ways on this Application:

• From you
• By using one of our partners services accessible through this Application.
• Accessing The Application through another site (also referred to as single sign-on), including employer, health plan or insurers websites or mobile applications.
• If we provide user account access, you may elect to establish an account so that you can gain additional access to online service applications, health tools, health information, subscriptions or other services where it is important for us to know who you are in order to best meet your needs. Providing Personal Information is always voluntary; however, certain services related to the Application will not be available if you do not create an account and provide the minimum necessary Personal Information to allow the account to provide the services you have requested.
• We may use “cookies” to help us improve this Application by tracking your navigation habits and to store some of your preferences. A cookie is a small file created by a website or application to store information on your computer. Cookies do not allow websites or applications to gain access to other information on your computer. Once a cookie is saved on your computer, generally only the website or application that created the cookie can read it.
• An Internet Protocol (IP) address is a number that automatically identifies the computer or mobile device that you are using to access the Internet. The IP address enables our server to send you the site pages that you want to visit or the data you want to view. The IP address may disclose the server owned by your Internet Service Provider. We use your IP address to help diagnose problems with our server and to support our administration of this Application.

How will information be used?

Any Personal Information that you provide is used for the purposes of providing our Application and related services to you.

Your information will be used for the following activities to the extent permitted or required by law:

• to allow Us to provide services to you related to the Application and to improve the Application user experience and provide more enhanced services and accuracy
• data for research and analytics, the results of which may published or otherwise shared
• population health analysis, public health activities and/or to respond to requests from relevant government health agencies
• to be disclosed to our service vendors, their sub-vendors, and Medicaid-Indiana to allow the correct functionality of the services under the Application and to facilitate the provision of the services under the Application to you
• if you are a member of a health plan we support, and if you’ve given us your permission, we may share information about you and/or your dependents with your health plan, including your group health plan

We may offer opportunities to provide tailored or customized services through our Application and partners upon your consent.

If you elect to establish an account we will retain your user ID and password in order to recognize you when you next log in.  Establishing a user account and providing Personal Information is always voluntary, however providing Personal Information through this Application is necessary for you to use its features.  If you do not wish to have your Personal Information collected you should not use the Application.  PLEASE NOTE:  If you use the Application without logging in, any Personal Information you enter into the Application may be accessible to anyone else who has access to your mobile device.  We recommend that you log in every time you want to use the Application. Remember, if you have accessed the Application through single sign on or otherwise from another application that shares your Personal Information with us, we may collect your personal information, even if you do not create a separate account in this Application.  

We may also gather quantitative user information, such as the number of users and the pages or data accessed, in order to perform administrative, technical, hosting or other functions that help us manage our Application and deliver new functionality to you. We do not sell, license, transmit or disclose Personal Information that you provide to us to third parties except with the following exceptions:

• Upon your authorization
• When such disclosure is necessary to allow us and our contractors or agents to carry out necessary functions to support the provision of the Application to you
• When required or permitted by law

Third parties

We may work with third party service providers who may place third party persistent cookies, web beacons, or similar technologies to collect anonymous information about the use of our Applications. They are not permitted to collect any Personal Information, and this information will be solely used for web usage analysis for a better understanding of how you use our Application, and/or to customize our content and advertising.

What if I don’t want information about me to be collected?

Providing Personal Information through this Application is necessary for you to use its features. If you do not wish to have your Personal Information collected, you should not use the Application.

“Do Not Track” Signals

We do not respond to web browser “do not track” signals. As such, your navigation of our Application may be tracked as part of the gathering of quantitative user information described above. If you arrive at our Application by way of a link from a third party application that does respond to “do not track” requests, the recognition of any “do not track” request you have initiated will end as soon as you reach our Application.

Communicating With Us

We may email, send push notifications and/or text information to further facilitate communication. Information collected through email or text may be shared with our Customer Service department, other associates, or third parties that perform services on our behalf. Unless otherwise noted, communication with us through our Application is not a completely secure and confidential means of communication. Non-encrypted email may be accessed and viewed by other Internet users without your knowledge and permission while in transit to us. Push notifications and/or texts may be accessible by anyone with access to your phone. Please do your part by using discretion and taking steps to secure your personal devices.

From time to time we will provide links to websites or applications not owned or controlled by us. We do this because we think the information might be of interest or use to you. A link to a third party website or application does not necessarily constitute or imply endorsement by us.
